Waiver
I do hereby release Faerieworlds LLC and any and all other officers, employees, volunteers, agents, insurers and any elected or appoint officials of said Faerieworlds LLC, individuals or entities affiliated with such persons and/or entities, from any and all civil liability or any and all forms of injury which may arise as a result of my participation in the Faerieworlds Festival on June 17-19, 2022.
I acknowledge and understand that there are many known and unknown dangers and/or risks associated with me participating in this event and I grant a general release, for myself, my heirs, executors, administrators and assigns and I waive, remise and forever discharge and release Faerieworlds LLC and any and all elected or appointed officials of Faerieworlds LLC, and all officers, employees, volunteers, agents, insurers, vendors, event participants and any other individuals or entities affiliated with such persons and/or entities from any and all claims, several or otherwise, pas, present or future, which can or may ever be asserted as a result of any and all injuries or damages, physical or mental, sustained by me while I am participating in Faerieworlds in any way, including my coming and going from such event.
I further understand that my participation in the aforementioned event does not create any form of association or employment with Faerieworlds LLC or any of its officers or employees and does not grant me any rights that are not expressly provided for by law or contract.
I hereby state and acknowledge that I am serving in a volunteer capacity by my own choice and accept the terms of my service as expressed in this document and I am not entitled wages, pay, or the usual benefits of employees of Faerieworls LLC for the services to be rendered to the event. I further represent that I am not currently employed by Faerieworlds LLC. I have read the foregoing and understand and agree and accept that the terms of this agreement are contractually and legally binding and that no verbal statement to the contrary, by any person or entity, can void or alter the terms of this agreement.

My child is hereby authorized and has my permission to serve as a volunte and participate in the Faerieworlds Event being held June 17-19, 2022.
I, the parent/guardian of the above named minor, for myself and on behalf of my child; acknowledge that my child's participation in this activity may involve the risk of injury form their actions, inactions, or negligence; from the actions, inactions, or negligence of others; from the conditions of the items being used, the weather or other occurrences in the area where the event is being conducted.
I release, waive, discharge, and relinquish the sponsors, organizers, and participants, their officers, directors, employee, and agents, from any and all liability, loss, damage, claim, demand, or cause of action against them, arising out of or related to my child's participation in this activity as a volunteer.
I assume all risks of bodily injury to my child and give permission for him/her to be taken to a hospital and/or treated by licenses medical personnel for a medical emergency, illness, or injury; and for licensed medical staff to take emergency measures as they deem appropriate.
I also understand upon arrival to the event, I will be asked to fill out another waiver that includes an emergency contact form for my child.
I have read this waiver and understand that it has legal consequences, and sign it voluntarily.
